Jolly Nanda had this to say about her experience with the accelerator: “I greatly value having had the opportunity to participate in the AHA EmPOWERED to Serve Business Accelerator in Minnesota. It helped me refine my pitch. I hope we will be able to leverage the support to raise awareness and funds to continue our mission. It was a pleasure to work my fellow participants and my congratulations to Kelly Robinson of Messianic Care PLLC on winning 1st Place, Michael Chaney of Project Sweetie Pie for Project Sweetie Pie on winning 2nd Place and Connie Sheehan of PUSH. I appreciate being selected as the Fan Favorite and thank all of those who voted for Altheia. My gratitude to the American Heart Association and the Cheerios team.”
Altheia is a cloud based, highly scalable platform targeted at small/mid-sized self-funded employer groups, health management companies and managed care companies. Altheia can ingest multiple formats for claims, chart and clinical data from any stakeholder within the healthcare ecosystem to enrich the member data with their consent. It provides highly accurate chronic disease risk prediction and segmentation to members and with their consent, to their care management team, allowing for the prioritization and timeliness of care delivery. We also provide full transparency, so members and care mangers understand what is behind the predictions. We focus on 5 of the costliest chronic diseases that can be managed, prevented from getting worse or reversed based on preventative care. Hypertension, Heart Disease, Diabetes, Chronic Kidney Disease and COPD. We also factor in SDoH to create a more accurate and personalized predictive model for each member. Our mission is to democratize access to disease management to those underserved groups that need it the most and provide member mobility within the healthcare ecosystem.
